  7. Line 11 - Sum lines 9 and 10 to find the total amount due. Payments must be made by Electronic Funds Transfer (EFT) Credit.
  8. Complete the Certification section by providing required signatures and additional details for the taxpayer and preparer. Ensure all information is accurate and complete.
