PRIMARY PURPOSE: The Physical Activity Risk Factor Questionnaire (PARFQ) is a self-screening tool required of all Navy members prior to participating in the semi-annual Physical Fitness Assessment (PFA).

Authorized Medical Department Representatives (AMDR). The AMDR must be a physician, adult nurse practitioner, physician assistant, or Independent Duty Corpsman (IDC) and must be appointed in writing by the unit CO or Navy Medicine Readiness and Training Command (NMRTC) CO.

Sailors will be able to view and access their personal PFA data via MyNavy Portal, on the MyRecord link and through the MyNavy Portal Quick Links.

Click on the Career & Life Events item at the top and select “Performance.” On the left-hand side, click “Physical Readiness,” then select “PARFQ” from the dropdown options. On this page, you can see your personal information.

Both male and female sailors 20 to 24 years old must hold in forearm plank position for at least 1:25 minutes. Males must complete at least 42 push-ups, and complete the 1.5-mile run in 13.5 minutes or less. Females must complete 17 push-ups and finish the 1.5-mile run in 15.5 minutes or less.
