1. Press the ‘Get Form’ button to access the Clearance Form and open it in your preferred editor.
2. Begin filling in your personal information at the top of the form. Provide your complete name, student ID number, phone number, permanent mailing address, and email address. Make sure all information is accurate and legible.
3. Sign the form where indicated for your student signature, and ensure that your parent or guardian also signs in the appropriate section.
4. Proceed to each department section listed. For each department, confirm that you have fulfilled their requirements: confirm all textbooks and library items have been returned, any fines have been cleared, and that all necessary signatures are collected.
