The Bournemouth questionnaire (BQ) is a comprehensive multi-dimensional core outcome tool assessing patients' outcome of care in a routine clinical setting. it is a short, self-report questionnaire, developed by J. Bolton.

The items of the questionnaire are assessed on a 0–10 numeric rating scale in which 0 means no disability and 10 is maximum disability. The sum of the seven items equals the total score of the PDI, which ranges from 0 to 70, with higher scores reflecting higher interference of pain with daily activities.

The Neck Disability Index (NDI) is a 10-item questionnaire that measures a patient's self-reported neck pain related disability. It was the first of its kind when it was published in 1991 in JMPT and was based on the Oswestry Low Back Pain Disability Questionnaire. The NDI was reviewed in 2008 by the same author.

Among those neck problems that qualify for disability per the SSA guidelines are degenerative disc disease (DDD), herniated discs, arthritis, whiplash, cervical spondylolisthesis, cervical retrolistheses, pinched nerves, cervical lordosis, and cancer.

The most used neck disability questionnaires are the Neck Pain and Disability scale (NPAD) and the Neck Disability Index (NDI). The Neck Pain and Disability scale (NPAD) is a composite index including 20 items which measure the intensity of neck pain and related disability.

Instructions to the client and scoring The WDQ takes only 5–10 minutes to complete and score and requires no special training to administer. Clients circle a number on an 11- point scale (0–10) for each item anchored by descriptors at each end-point. Scores for each item are added to give a total score out of 130.

The Back Bournemouth Questionnaire is used mainly for comparative purposes. The first step is to obtain a Baseline Score which is best done on the day of the initial examination. This is done by summing up the 7 individual scores (total maximum score = 70). This score is then later compared with Subsequent Scores.
