7. If the vehicle has a current safety inspection sticker, indicate this and fill in the safety inspection sticker number if applicable.
8. You will need a law enforcement officer to verify the safety inspection information in the designated section if applicable.
9. Ensure to include a notarized affidavit and any additional required documents such as verification from a Pennsylvania State Police Vehicle Fraud Investigator.
