- In Part Four: Self-evaluation, reflect on your experience by addressing the eight learning outcomes. Check off which outcomes were achieved and describe how you achieved each outcome thoroughly, using separate paragraphs as necessary. It’s important to clearly label each outcome by its number.
- Finally, in Part Five: Final verification, have your homeroom teacher sign the form. Ensure that you return the completed form within 30 days of completing your activity. If the activity continues beyond this period, make a copy of the form, note 'See attached log' on the front, and attach the continuation log to submit monthly until the activity concludes.
