Infection surveillance definitions for long-term care facilities (ie, the McGeer Criteria) have not been updated since 1991.

Must fulfill at least 2 criteria Runny nose or sneezing Stuffy nose or nasal congestion Sore throat, hoarseness, or difficulty in swallowing Dry cough Swollen or tender glands in the neck (cervical lymphadenopathy) Fever many or may not be present. Symptoms must be new and not attributable to allergies.

At least one of the following sign/symptom sub-criteria present: Fever, rigors, or new-onset hypotension with no alternate site of infection. Either acute change in mental status or functional decline, with no alternate diagnosis AND leukocytosis. New-onset suprapubic pain or costovertebral angle pain or tenderness.

McGeer and NHSN Criteria are Designed for Surveillance Revised McGeer criteria (Stone 2012) are used for retrospectively counting true infections. To meet the criteria for definitive infection, more diagnostic information (e.g., positive laboratory tests) is often necessary.

The diagnosis should be confirmed by urinalysis with examination for pyuria and/or white blood cell casts and by urine culture. Urine cultures demonstrate more than 100,000 CFU per mL of urine in 80 percent of women with pyelonephritis. Blood cultures are positive in up to 20 percent of women who have this infection.
