6. For the 'Organization’s Activities' section, list the main sources of funding for your organization's activities. Provide details on any activities organized related to the United Nations and include information about your UN-related newsletters, magazines, or websites.
7. In the References section, list four organizations that are familiar with your work, ensuring to include their names and complete contact information.
8. At the conclusion of the form, input the name of the President or Chief Administrative Officer completing the application, and secure the signature along with a stamp. Finally, ensure to date the form.
