8. Optionally, provide your email address to receive notifications related to contractor training events and legal updates.
9. Indicate whether you, your spouse, registered domestic partner, or principal owners have been registered as contractors before. Answering 'No' when you have been previously registered may result in fines.
10. Indicate if your company will continue to conduct business under prior registration numbers.
11. If you will have employees, confirm whether you have applied for an Industrial Insurance Account Number.
12. Select the Contractor Type—General Contractor, which allows work across multiple trades, or Specialty Contractor, limited to a single trade.
13. If you selected Specialty Contractor, write the corresponding specialty code from the provided trade types list.
14. Complete the personal information section for each owner or officer, including Full Legal Name, Social Security Number, Date of Birth, Driver’s License Number, Residence Address, and Title.
15. Affix signatures in front of a notary for all individuals listed in the application, certifying that all information is accurate.
16. Once all sections are filled out and reviewed, you can save changes, download the completed form, print it, or share it as necessary.
