- Sign and date the application to certify the accuracy of the information provided and that you meet the requirements under 18 USC 926C.
- Ensure you have included all required documents with your application, including a photocopy of your photographic law enforcement identification, and a letter from your former agency verifying your employment status.
- Prepare your application processing fee of $20 by obtaining a money order or cashier’s check payable to the Arizona Department of Public Safety.

LEOSA defines, Qualified Retired Law Enforcement Officer, as LEOs who are retired or separated from CBP or its predecessor agencies, and meet all LEOSA criteria, (see What is a Qualified Retired Law Enforcement Officer? ). Qualified retired LEOs also include CBP LEOs who converted to a non-LEO position.
