- If this is your first time applying for HUT credentials, fill in Line 12 with information about all corporate officers, members, or partners as required.
- In Line 13, provide details about the custodian of your business records, including their address and phone number.
- Line 14 must be completed if a non-officer is preparing the application, including a power of attorney if applicable.
- Ensure that the application is signed by the appropriate party as noted in Line 15.
- Line 16 requires you to enter the number of credentials you are ordering and multiply it by $1.50. Attach the payment for this amount.
- Complete vehicle information in Line 17, marking all relevant columns from A to L correctly. If you need more space, attach an additional sheet.
