Each item is rated on a five-point scale with 0 indicating normal mood and behavior and “4” indicating severe impairment. The total score can be used to reflect the severity of mania as either mild (15–20), moderate (21–28), marked (29–32), severe (33–43), and extreme (≥44) (Bech, Baastrup, de Bleeker, & Ropert, 2001).

Items are rated on a frequency (0–5 scale, range 0–130) and severity scale (1–5 scale, range 26–130), with higher scores indicating greater severity of manic symptoms.

A score of 20 is usually required for the YMRS (Tohen et al., 2000). However, this threshold of 20 is rather arbitrary and has not been formally validated.

Severity ratings are based on the patient's subjective report of his clinical condition during the past 48 hours and the clinician's observations during the interview. YMRS total score varies between zero and 60. This scale has shown good psychometric properties.

The DSM-5 Level 2—Mania—Adult measure is the Altman Self-Rating Mania Scale. The ASRM is a 5-item self- rating mania scale designed to assess the presence and/or severity of manic symptoms. The measure is completed by the individual prior to a visit with the clinician.

The YMRS total score ranges from 0 to 60 where higher scores indicate more severe mania, thus, a negative change (or 50% reduction) from baseline indicates a reduction (or improvement) in manic symptoms.

The YMRS follows the style of the Hamilton Rating Scale for Depression (HAM-D) with each item given a severity rating. There are four items that are graded on a 0 to 8 scale (irritability, speech, thought content, and disruptive/aggressive behavior), while the remaining seven items are graded on a 0 to 4 scale.

The YMRS total score ranges from 0 to 60 where higher scores indicate more severe mania, thus, a negative change (or decrease) from baseline indicates a reduction (or improvement) in manic symptoms. Total score ≤12 indicates remission (13-19=minimal symptoms; 20-25=mild mania, 26-37=moderate mania, 38-60=severe mania).
