- Select your payment method: cheque, money order, or credit card. If using a credit card, fill in the required details including card number, expiry date, and name on the card.
- Sign and date the form where indicated, confirming your details.
- Finally, save your changes, download the completed form, print it, and share it via the provided methods: mail to the Membership Department or fax it.
