The Brief Interview for Mental Status (BIMS) was introduced as part of the Minimum Data Set 3.0 to measure cognitive impairment in nursing home populations.13,14 The BIMS is a brief cognitive screening measure that focuses on orientation and short-term word recall.

Creating the Cognitive Function Scale Residents with a BIMS score of 8–12 or a CPS score of 0–2 were considered to be “mildly impaired.” Residents were considered “cognitively intact” if they were able to complete the BIMS and scored between 13 and 15.

Each of the seven items that BIMS consists of, is awarded a number of points ranging from 0 to 3. The higher the score, the lower the impairment to the cognitive response. Scores closer to 0 indicate severe cognitive impact whilst scores closer to 15 indicate an intact cognitive response: BIMS score.

13 to 15 points: intact cognition. 8 to 12 points: moderately impaired cognition. 0-7 points: severely impaired cognition.

The total BIMS score ranges between zero to fifteen points and is categorized into three cognitive groups: Intact, Moderate, and Severe.

The BIMS is a simple test where a clinician says three words and asks the patient to remember and repeat them. For example, they would say, “sock, blue, bed.” S/he may repeat these same words using cues. For instance, CMS says, a sock is something you wear, blue is a color, and a bed is a piece of furniture.

The Brief Interview for Mental Status (BIMS) is used to identify delirium and needed supports in patients living in skilled nursing facilities (SNFs) and long-term care facilities (LTCFs).
