Along with filling out the Form 602, compile all supporting documents listed on the form. You MUST also include both a copy of your CDCR Form 1515 AND a copy of your Form 22 as supporting documents. When an inmate (serving an indeterminate sentence) becomes eligible for parole, he or she is given a tentative hearing date for a parole suitability hearing. The hearing is then automatically set one year prior to an inmate's minimum eligible parole date.
