12. If you want to add or delete a plan, fill out the ‘Plan Addition/Deletion’ section accordingly.
13. For check/cash and credit card conversions, indicate your choices in the designated section, providing your updated credit card number and expiration date.
14. Sign in the ‘Required Signature’ section to authenticate your updates.
15. Fill in the ‘Date Account Updated’ field with the current date, plus the initials of the updating CSR.
