Coding Information The evaluation of drugs for addition to the self-administered drug (SAD) list is an ongoing process. This list contains only those drugs and biologicals that are determined to be “usually self-administered by the patients” and therefore not eligible for Medicare coverage.

At times, outpatient hospital treatments require patients to use self-administered prescription drugs or biologicals, such as Tylenol. Drugs or biologicals are deemed “self-administered” because the patient can take them or use them without assistance from health care industry professionals.

Policy: Self-administration of medication is defined as the preparation and administration of medication to self, by a patient, under the direct supervision of a nurse. A Physician, APRN/PA may write an order to allow a patient to self-administer an Epi-pen or Inhaler without direct supervision by the Nurse.

Other routes of administration including, but not limited to, oral drugs, suppositories, topical medications are all considered to be usually self-administered by the patient.

Medicare Drug Coverage Medicare defines self-administered drugs as medication the patient could, in another setting, take him or herself. Medications brought from home in their original pharmacy container, properly labeled and positively identified may be used with a physician order.

The Medicare program provides limited benefits for outpatient prescription drugs. The program covers drugs that are furnished "incident-to" a physician's service provided that the drugs are not "usually self-administered" by the patient.

Drugs Given in Hospital Outpatient Settings “Self-administered drugs” are medications that you would normally take on your own, like medications that you take every day to control blood pressure or diabetes. In most cases, Part B generally doesn't pay for self-adminstered drugs used in the hospital outpatient setting.

'Self administration' is when a person can take their own medicines. People have the right to choose to manage their own medicines. Staff should consider a person's choice and whether there is a risk to them or others.
