- Section 5 includes payment instructions. Decide if you want your proceeds via direct transfer or cheque. If opting for direct transfer, fill in the bank/building society name, branch sort code, branch address, bank account number, and account holder name. If you choose a cheque, tick the box indicating that preference.
- Finally, in Section 6, provide your signature as the first holder, along with the date. If applicable, joint holder must also sign and date the document.
- After completing the form, review all entries for accuracy. Save any changes, and you will have the option to download, print, or share the filled form.
