Students looking for transcripts from closed institutions should go to the Ohio State Board of Career Colleges and Schools website. This website lists closed colleges in Ohio, along with additional information about how to request a transcript for each closed institution.

To obtain a copy of your High School Transcript, contact the school district from which you graduated. Enclose a certified check or money order payable to the New York State Education Department: $4.00 for a copy of a transcript (only passing transcripts result in a diploma) $10.00 for copy of diploma and transcript.

Electronic transcripts must be transmitted from a secure site formally linked to the sending institution. Graduate Applicants: If an email address is required, please instruct the transcript service to email the transcript to etrans@usc.edu.

All inquiries including graduation verification, transcript, shot records, IEP, ETR, and cumulative folder must be requested online using our new online student record request system. Fax, phone calls and mail will no longer be accepted.

Students can acquire a copy of their transcripts from their local school, school district or the Louisiana Department of Education. Schools and school districts maintain permanent records and provide electronic copies to the Louisiana Department of Education via Student Transcript System (STS).
