Home Language Survey California Education Code, Sections 313 and 60810 contain legal requirements which direct schools to determine the language(s) spoken in the home of each student. This sample form is designed to assist with the identification process.

The responses to the home language survey will assist in determining if a student's proficiency in English should be tested. This information is essential in order for the school to provide adequate instructional programs and services.

The home language survey (HLS) is a questionnaire given to parents or guardians that helps schools and LEAs identify which students are potential ELs and who will require assessment of their English language proficiency (ELP) to determine whether they are eligible for language assistance services.

For questions related to the HLS, contact the CDE Language Policy and Leadership Office at 916-319-0845. If the Home Language Survey is completed in error, the parent or guardian may make a request to change it.

At any time you may request that your child opt out of the ESL program. It will be noted that your child should not receive direct ESL services. However, your child will still be considered LEP, and based upon his proficiency levels, may still qualify for testing accommodations.
