7. In the Beneficiary Nomination section, nominate the person you wish to be the beneficiary. Include their first names, surname, identity number or passport, and relationship to you.
8. Moving to Payment Details, select a mode of payment and provide the frequency of payment, the policy commencement date, and the details of your family cover and premiums totals.
9. Enter your Bank Details, including the name of the bank, branch code, branch, account number, account holder's name, and type of account. If you're opting for a debit order, choose the deduction day.
10. Complete the Compliance Checklist by indicating 'Yes' or 'No' where applicable for various disclosures made to you during the application process.
11. Sign the declaration by the Proposer/Premium Payer, ensuring all information is accurate before submitting the form. Follow any instructions related to the intermediary’s declaration if applicable.
12. Finally, once all sections are filled out, save any changes made to the document. You may download, print, or share the form as required.
