14. Make the required changes in the Transportation Supplement screen and ensure to SAVE your progress.
15. Click 'Return' to go back to the Amendment screen.
16. Input the names of the parent and the teacher in their respective fields.
17. Ensure to obtain the principal's signature as it is mandatory.
18. Once completed, remember to click on 'SAVE.'
19. Use the Reports feature to create and print the document, ensuring to attach the amendment form to the transportation form.
20. File both forms in the eligibility folder and send a copy to the parent.
21. Finally, archive the report created that includes both the Amendment form and the Transportation Supplement.
