Medication evaluation or medication-use evaluation (MUE) is a quality assurance (QA) tool. It reviews a patient's prescription and medication against a set of standards. A multidisciplinary team, comprising physicians, pharmacists, nurses, and administrators, prepares these standards.

Medication review involves an evaluation of a patient's medicines with the aim of optimising the quality use of medicines. A medication review will often result in the identification of actual or potential medication-related problems and recommendations to optimise medicines use.

The MAR is a required document – it must be used for any APD client who is receiving assistance with their medications! Sometimes you will use a MAR provided by a pharmacy, or you will document medications electronically using a computer.

The list should include the name of the medication, the dose, and the number of times a day you have to take it. Include information about how to take the medication (with or without food, as a pill, as a shot). Include information about any allergies. Share the list with close friends, family, and caregivers.

The times and dates the medication is to be taken 3. The initials of the person assisting with the medication 4. A start date should be noted; a stop date is noted when known 5. Identifying information about the individual, including date of birth, allergies, diagnoses, and names of medical providers.

During a medication review, the patient will provide a doctor or nurse with detailed medical information. The patient reveals all present illnesses and any supporting prescription drugs. Patients should also include non-prescription medicines, supplements, and homeopathic treatments.
