A complete physical examination includes inspection and palpation, assessment of range of motion and strength, and provocative shoulder testing for possible impingement syndrome and glenohumeral instability.

The examiner places the patient's arm shoulder in 90 degrees of shoulder flexion with the elbow flexed to 90 degrees and then internally rotates the arm. The test is considered to be positive if the patient experiences pain with internal rotation.

The patient performs active movements in all functional planes for the shoulder. This includes flexion, extension, abduction, adduction and internal and external rotation.

The special tests described in this review evaluate specific tendons of the rotator cuff. The Jobe's test and drop arm test evaluate the supraspintus whereas the lift-off test, passive lift-off, and external rotation lag signs assess the infraspinatus and teres minor.
