4. Fill out the entire user information, including first name, last name, email address, phone number, shipping location details, and any additional comments. Ensure that no abbreviations are used.
5. If applicable, indicate whether the shipping location contact is the same as the user by selecting ‘Same as User’ or provide different contact information.
6. Once all information is completed, save the form. Ensure you save it as a Microsoft Office Excel 97-2003 Workbook and name the file in the format: 'Parent Vendor Number'-'Vendor Name' (e.g., 176521-Fossil Geode).
7. Submit the completed form via email to transportation@steinmart.com at least three business days prior to requesting routing through SmartRoute.
