- In Section 4, if you are replacing someone, input their name and CTEP ID. Specify whether the individual being replaced remains an active member. If they are inactive, they will be removed from the site roster.
- Section 5 requires approvals. Indicate yes or no to verify if approval is needed. Obtain signatures and print names for the Lead RA or Co-Lead RA and the Contact PI if applicable, alongside their respective dates.
- Once all sections are completed, review the entire form for accuracy. After that, you may save changes, download, or print the form to finalize your submission.
