The CDR is a 5-point scale used to characterize six domains of cognitive and functional performance applicable to Alzheimer disease and related dementias: Memory, Orientation, Judgment & Problem Solving, Community Affairs, Home & Hobbies, and Personal Care.

The Dementia Rating Scale (DRS) is considered a very useful instrument to assess patients with dementia. The tasks are grouped into five subscales, each one evaluating different cognitive areas, namely: Attention, Initiation/Perseveration (I/P), Construction, Conceptualization and Memory.

DSM-IV criteria for the diagnosis of dementia require the presence of multiple cognitive deficits in addition to memory impairment6 (Table 1). Early in the disease, memory impairment may be the only clinical finding, and this single finding would not meet the diagnostic criteria for dementia.

Scores on the MoCA range from zero to 30, with a score of 26 and higher generally considered normal. In the initial study data establishing the MoCA, normal controls had an average score of 27.4, compared with 22.1 in people with mild cognitive impairment (MCI) and 16.2 in people with Alzheimer's disease.

A score of 20 to 24 suggests mild dementia, 13 to 20 suggests moderate dementia, and less than 12 indicates severe dementia. On average, the MMSE score of a person with Alzheimer's declines about two to four points each year.

Most cases of sudden confusion and rapidly progressing dementia in an elderly person are due to delirium caused by infection. Urinary infections and pneumonia can trigger acute confusion that comes on quickly, causing people to be incoherent, muddled and disorientated.

There are five stages associated with Alzheimer's disease: preclinical Alzheimer's disease, mild cognitive impairment due to Alzheimer's disease, mild dementia due to Alzheimer's disease, moderate dementia due to Alzheimer's disease and severe dementia due to Alzheimer's disease.
