Follow the steps to complete your Eavpay registration agreement.

1. Press the ‘Get Form’ button to access the registration agreement for Eavpay and open it in your preferred document viewer.
2. Fill out Section 1 with your registrant information, including organization name, contact person, address, and contact details. Ensure all fields are completed accurately.
3. In Section 2, designate users assigned to this registration. Clearly identify each user as either an 'Authorized User (AU)' or a 'Data Entry User (DEU),' ensuring at least one AU is included.
4. Provide the necessary details for each designated user, including their email addresses and Provider ID. If applicable, indicate the location/extension for multi-site providers.
5. In Section 3, review the agreement terms before signing. Confirm that you have included all required information and designated users as outlined.
6. Sign and date the registration agreement. Ensure your email address is provided, as usernames and passwords will be sent to the address listed in Section 1.
