1. Click the ‘Get Form’ button to obtain the Intent to Enroll form and open it in your online editor.
2. Enter your last name in the designated field.
3. Provide your first name and middle initial in the required fields.
4. Fill in your date of birth in the format MM/DD/YYYY.
5. Enter your UH number, which is your student ID number.
6. Complete your address, including city, state/province, zip/postal code, and country if applicable.
7. Indicate your acceptance of the offer of admission by selecting the semester in which you will enroll: Fall or Spring.
8. If you accept the offer, complete the tuition deposit section, noting that it is non-refundable and non-transferable.
9. Select one tuition deposit payment option: a money order or check from a US bank, federal financial aid deduction, or credit card payment. If you choose credit card, proceed to the following steps.
10. If paying by credit card, fill in the cardholder’s name, phone number, and billing address.
11. Select the card type and provide the card number and expiration date in the given fields.
12. Acknowledge your agreement to pay the tuition deposit as per the card issuer agreement by signing below.
13. Finally, submit the completed form and payment to the UH Mānoa Office of Admissions by mail or contact them if you require assistance.
