10. Complete the system specifications, including tilt, orientation, total system size in kW, customer's annual electric usage in kWh, expected annual kWh produced, and annual electrical offset.
11. If applicable, indicate the battery capacity, and check the box if the total square footage of the PV array exceeds 4,000 square feet for an environmental assessment.
12. In the certification statement section, ensure the installer or contractor provides their signature and date, along with printed names. Obtain and provide the customer's signature and printed name as well.
