2. In Section 1, complete the fields required from the pensioner. This includes entering your full name, Social Security number, home address, city, state, zip code, and phone number. Indicate whether this is a change of address by selecting 'Yes' or 'No'.
3. If you are using a checking account, attach a deposit slip or a voided check to your submission. Be aware that for credit unions, a voided check is necessary.
4. Sign the designated area in Section 1 to authorize the Laborers' Pension Fund and your financial institution to process the direct deposit. Include the date of your signature.
5. If applicable, Section 2 must be completed by any joint account holder(s). This will involve their consent to the direct deposit instructions. They should also include their signature and date.
6. Section 3 is to be completed by your financial institution. Provide the name, location, street address, transit/ABA number, and account number. Ensure that the name(s) on the account are listed, and obtain the signature of an authorized officer from the financial institution along with the date.
7. Once all sections are filled out and verified, return the original completed form to the Laborers' Pension Fund while keeping copies for your records. Save, download, or print the form for your future reference if necessary.
