- Begin with Part 1 by entering the main contractor’s particulars. Fill in the CPF Submission Number, the name and address of the main contractor, and provide the necessary contact details including postal code, telephone number, fax number, and handphone number.
- Proceed to Part 2 to enter project details. Provide the location, description, commencement and completion dates, original contract value, date of the letter of award/contract, BCA/BCU approved plan number, and the type of project. Ensure to include the developer's and architect's contact details.
- In Part 3, the architect or qualified person (QP) must declare accurate information about the project. They need to confirm original and remaining contract values, including their name, designation, and ID.
- In Part 4, the professional engineer or QP repeats the declaration process, providing similar details as in Part 3.
- Part 5 involves a declaration from the developer or owner, confirming their role and providing correct contract values.
- Finally, in Part 6, the main contractor must complete their declaration with the same format as previously. They must also recognize if their company is a partnership and sign accordingly.
- After completing all parts, ensure to check if all information is provided accurately before submitting. Your completed application form, along with required documents, should be mailed as specified in the key information section.
- Once submitted, wait for the processing time of 12 working days, during which the possibility of needing to present original documents may arise. You will receive the outcome by mail.
