8. Sign the form to agree to the liability release and date your entry. Also, print your name clearly below your signature.
9. If you reside in Bell County, check the appropriate box and provide the date of birth for any participants aged 12 and under.
10. Review all entries for accuracy and completeness before proceeding to save the document. You may download, print, or share the filled form as needed.
