  5. Repeat the process for any contingent beneficiaries in the same section, ensuring their total share percentage also equals 100%.
  6. If designating a living trust or testamentary trust, fill in the relevant details including the trust name and trustee information, ensuring to specify the shares for primary and contingent beneficiaries.
  7. For charity or organization beneficiaries, list the name of the charity, address, and share percentage. Do not use names of individuals associated with the organization.
  8. In Section III, review the signing requirements. If you are completing this form as an agent, check the appropriate box and include the Power of Attorney documentation.
  9. Finally, sign and date the form in the designated area stating you revoke any previous designations. Ensure all pages of the form are included.

General Instructions Write only one beneficiary on each line. Make sure that you write the full names of all beneficiaries. For example, if you name you children as beneficiaries, DO NOT merely write “children” on one of the lines; instead write the full names of each of your children on separate lines.
