4. Complete Section III by detailing the study history of the qualification holder. For each qualification, specify the name, awarding institution, country, duration of attendance, and whether it was completed.
5. For Section IV, indicate the reasons for needing the evaluation by checking the relevant boxes, such as for work visas or further study.
6. In Section V, declare how you would like the results delivered, either for pickup at SAQA offices or by post. Be sure to provide any necessary permissions for someone else to collect the results.
7. Complete Section VI to specify the services you require from SAQA, ticking the required options along with the corresponding fees.
8. In Section VII, calculate the total payment due to SAQA based on the selected services and include proof of payment with your application.
9. Ensure all statements are completed and review Section IX, the declaration. Sign and date it to confirm your understanding and compliance with all requirements.
10. Finally, review all sections for completeness, save your changes, and either submit the application as instructed or print a copy for your records.
