Follow the steps to complete your Sma Service Rebate form online

2. Fill in the Installer/Dealer Company Information section. Include the date, company name, installer’s name, company address, city, state, zip code, phone number, cell number, email address, and the date(s) of service calls.
3. Provide the Jobsites Information. Fill in the client name and jobsite address, along with the city, state, zip code, and phone number.
4. List each service call and corresponding rebate amount in the designated sections, ensuring you detail the date and initial of the person completing the form.
5. Complete the Authorization Agreement for Direct Deposits (ACH Credits) by entering the company name, email address, and selecting the type of account. Fill in your financial institution details, including name, city, state, zip code, account name, routing number, and account number.
6. Sign and date the authorization agreement to complete this section.
7. Complete the W-9 form with your taxpayer identification number and other requested information. Ensure all names and numbers are correct to avoid any issues.
