This form is to be used by employers and/or taxpayers to report essential information for the collection and distribution of Local Earned Income Taxes. This form must be utilized by employers when a new employee is hired or when a current employee notifies employer of a name and/or address change.

Form 8802, Application for United States Residency Certification is used to request a certificate of residency, Form 6166, that residents of the United States may need to claim income tax treaty benefits and certain other tax benefits in foreign countries.

Temporary Residence Certificate (TRC)

Form 6166 is a letter printed on stationery from the U.S Department of Treasury used as proof to foreign governments that U.S residents are eligible to receive tax treaty benefits. To receive a Certification of U.S. Tax Residency, you have to send in Form 8802 with the appropriate personal information to the IRS.

All Pennsylvania employees are required to file this Residency Certification Form with their employer. You should have received a form of a communication from your local tax municipal center with a 6 digit political subdivision code (PSD).

To obtain Form 6166, a letter of U.S. Residency Certification, you must submit a completed Form 8802, Application for United States Residency Certification. A user fee is charged to process all Forms 8802.
