- In Section I: Student Information, enter your full name, Student ID (if you have one), and your address including city, state, and zip code. If applicable, include your spouse’s name.
- Proceed to Section II: Income Received from January 1, 2013, to December 31, 2013. Fill in the yearly totals for all sources of income for both yourself and your spouse, ensuring no lines are left blank.
- Move to Section III: Living Expenses for the year 2013. This section is divided into four parts (a-d). Start with Section III-a: Housing Expenses. Select your housing type, indicate who paid for your housing, and provide the total housing cost and utilities expenses for that year.
- Continue to Section III-b: Personal Expenses. Document the total costs for food, phone expenses, and personal care items, indicating who paid for these costs.
- Proceed to Section III-c: Transportation Expenses. Select the type of transportation used and detail the expenses associated with transit, including car payments and insurance.
- Complete Section III-d: Medical Expenses. Answer whether you had medical insurance coverage and provide any associated costs, including co-pays and prescriptions.
- In the additional information space, include any relevant details that do not fit into the previous sections if necessary.
- Finalize your form by ensuring all sections are completed accurately. Review your information and save your changes. You can then download, print, or share the form as needed.
