The standard form can report the ballast water history of up to 20 tanks. The Extended Form can report up to 36 tanks. Purpose: The Coast Guard will use the information provided on ballast water reporting form to evaluate vessel ballast water management practices. BALLAST WATER HISTORY: Record all tanks to be discharged INSIDE US EEZ (in route to and at the port or place of arrival). BALLAST WATER HISTORY: Record all tanks to be deballasted in port state of arrival (enter additional tanks on page 2).
