9. Name your regular doctor and provide their address.
10. Indicate if you are insured for similar benefits with another company. If yes, include the name of the insurer, the amount insured, and any amounts compensated.
11. Complete Part B, Declaration and Authorisation. If the insured person is under 21, ensure a parent or legal guardian signs this section.
12. Read and understand the consent regarding the collection, use, and disclosure of personal data as outlined in the form.
