Get St. Bonaventure University Transcript Request Form - Sbu

Transcripts may be ordered in person or by mail by completing the Transcript Request form available in the Registrar's Office or online. In Person request for transcripts can be made at the service counter of the Registrar's Office located in Meisler Hall, Suite 1100.

Official transcripts should be sent electronically to spd_transcripts@stonybrook.edu directly from the issuing institution. Do not send transcripts to the Graduate School or Undergraduate Admissions. Electronic test scores should be sent to SUNY Center Stony Brook University Code 2548.
