To change your name and/or address for one of the following professions please visit www.michigan.gov/miplus: For all other health professions you must submit a request to change your name in writing.

There is no fee for a change of address. Other corrections must be submitted at a branch office and include changing your name or date of birth, or adding and dropping endorsements and Commercial Driver License group designations. Fees will be based on the license type.

If you have an address change, please make the change and renew your license at www.michigan.gov/elicense. If you have a name change, please submit your name change to our office at bplhelp@michigan.gov.
