There is an application to be completed to obtain an account number, however there is no fee for the application.

You must apply to the Office of the City Comptroller by submitting a notarized affidavit and two proofs of identification. Acceptable forms of identification include a voter registration card and copies of both the State and City tax returns.

You obtain a Certificate of Residence FROM THE COUNTY of your permanent address by providing proof of residency. It is recommended that you contact your County's designated office for information on how to obtain a Certificate of Residence. Refer to the List of New York State County & Local Offices.
