Sign in to Retirement Online and click “View and Update My Beneficiaries.” You can add beneficiaries, update beneficiary information, or remove beneficiaries. Complete the Designation of Beneficiary form (RS5127) and mail it to NYSLRS. (This form is for active members only.

For deferred compensation plan, in the event that you pass away, your beneficiaries essentially step into your shoes and have all the same rights that you did in the plan, including the right to start receiving payments or continue to defer them just like you had the right to do.

If you get married or divorced, or have children or other life changes, standard sequence will follow those life changes. If you never file a beneficiary designation, your benefit will be paid ing to standard sequence at the time of your death.

Write only one beneficiary on each line. Make sure that you write the full names of all beneficiaries. For example, if you name you children as beneficiaries, DO NOT merely write “children” on one of the lines; instead write the full names of each of your children on separate lines.

If you don't name a beneficiary as part of your life insurance policy, the death benefit will flow through your estate. Your loved ones will receive a smaller lump sum payment, and payment will be delayed as the funds go to probate as part of your estate.

The beneficiary designation forms allow you to name primary and secondary beneficiaries. Your “primary beneficiaries” are the first people or entities that you want to receive your benefit after you die.

Provide the following information on the beneficiary designation: The full name of the trust as it shows on the trust document. The date the trust was created. The name of the trustee, followed by the word “trustee”, or if you cannot provide a trustee, ETF may accept another contact person. The trustee's address.

Setting up a trust for them is the best way to proceed if you still want them to get your assets. Additionally, it's best to leave your estate out as a beneficiary because naming it requires probate to determine who gets what, which could take time, and there could be large tax consequences if you name your estate.
