9. Select the business development pack that you wish to purchase by checking the appropriate boxes and noting the prices.
10. Choose how you would like to receive your commissions, noting your preferred payment mode, such as cheque, credit on account, or another method.
11. Certify that all information provided is true and sign the form, recording the date of signature.
12. Attach proof of valid identification with a photo and send the completed form to the designated email or fax number, or deliver it in person to any Human Nature branch.
