Follow the steps to successfully complete your Sunycard Authorization Form.

1. Select the ‘Get Form’ button to access the Sunycard Authorization Form and open it for editing.
2. Begin by filling out the Cardholder Information section. Enter your address in the designated space provided.
3. Indicate your SUNYCard status by selecting one of the available options: Staff/Administrator, Full-time Faculty, Adjunct Faculty, Credit-Free Instructor, Credit-Free Student, or Community Borrower.
4. In the 'Type of Card Authorized' section, choose whether your card is permanent or if it has an expiration date. Ensure you circle the appropriate option.
5. Complete your personal details, including your name, city, state, zip code, and telephone number in the respective fields.
6. For identification, you can indicate if you have a Driver's License or if you are an alumnus. If you are a Community Borrower, please note the $5.00 fee is required.
7. Leave the sections marked for Authorizing Office only and Security Office use blank. These will be filled out by the appropriate staff after you submit your form.
8. Once you have completed the form, review your entries to ensure they are accurate. Save your changes and proceed to print or download the form as needed.
9. Take the completed form, along with the receipt for any applicable fees and your photo identification, to the Security Office (S-114) for the issuance of your SUNYCard.
