- In Section 1, Customer Service Request, select the type of request that applies to your needs, whether it be for establishing a new service, making changes, or cancelling existing services.
- Move to Section 2, Cash Services. Here, choose the specific service you wish to add or cancel from the available options.
- In Section 3, Financial Institution Information, enter the new or current branch details. Make sure to provide both the ship to and administrative addresses. When changing an existing service, fill in only the information that you want to update in the 'Change To' section.
- For Armored Carrier changes, refer to the instructions in the 'Carrier Name' fields. If your armored carrier also provides cash vault services, indicate the carrier name on the 'branch name' line and use their address for the street address.
- In Section 4, Authorized Signature from Official Authorization List, complete all requested information. It is crucial that the form is signed by an authorized officer listed on your Official Authorization List (OAL).
- Once the form is filled out completely, you can submit it by emailing it to the Customer Contact Center at ccc.bankservices@kc.frb.org, faxing it to (877) 281-3647, or mailing a signed hard copy to Customer Contact Center, P.O. Box 219416, Kansas City, MO 64121-9416.
- Lastly, ensure that you have provided a minimum of five days' notice for processing requests and check that you have entered the requested effective date at the top of the form.
