7. Complete the 'Company/Organization Name' section with the registered name and country of incorporation.
8. Fill in the 'Office Telephone No.', 'Mobile No.', 'Email Address', and 'Company or Organization’s website address' accurately.
9. In the 'Other Accounts currently held' section, list any accounts you have with DTB or other banks, including the bank name and branch.
10. Sign and date the 'Signature & Mandate Form' for each signatory as per the requirements detailed in the form.
11. Collect and attach the necessary documents indicated in the separate checklist for account opening.
12. After completing the form, review all details for accuracy. You may then save your changes, download, print, or share the form.
