Units, councils, and districts are required to submit Historian Reports annually, through channels. Complete this form (front and back) and make TWO copies. 3. Make 2 copies of your completed form: • Give 1 copy to your district secretary to file with the minutes. Every PTA is required to prepare an Annual Historian Report. Information from these reports is compiled and forwarded to the California State PTA.
