- If applicable, provide details regarding your Loan Out company, including name, federal tax ID, and address.
- Indicate if you wish to transfer your local affiliation by noting your current local and the local to which you wish to transfer.
- Complete the member authorization section by printing and signing your legal and professional names. If the member is a minor, include the signatures of both parents or legal guardian along with legal proof of guardianship.
- Once all fields are completed, save your changes, and prepare to submit the form via mail or fax to the designated membership services address provided.

There are three ways to qualify for SAG membership. 1 - Get hired as a principal performer in a SAG signatory show. Performers may join SAG upon proof of employment. Employment must be in a principal or speaking role in a SAG film, videotape, television program or commercial.
